I have read and agree to abide by the GymWaiver as outlined here Guardian- The Gym Waiver will also need to be signed in person in the presence of a parent/guardian when the fob is being collected. Please contact Jamie O’ Sullivan 087 134 5321 regarding this.


Consent

  1. 15-18’s can use the gym under a supervised vetted adult/guardian/coach only during designated times on the gym timetable.
  2. All members must have completed a gym waiver form.
  3. Return weights & bars after use.
  4. Equipment must not be removed from the premises at any time.
  5. Sanitize equipment after use.
  6. Turn off lights & ensure all doors are closed before you leave.
  7. No food or drinks allowed – apart from bottled water.
  8. Be considerate of others.
  9. Leave the gym clean & ready for the next session.
  10. Only members can access the gym with their own fob and under no circumstances can a gym member allow a non-gym member to use the facilities with their fob.
  11. If there is an accident/ incident, please notify club officer(s) immediately.
